What changed on October 1, 2025
For years Arizona providers could lean on the state-funded Sandata arrangement. EVV 2.0 flipped both the plumbing and the economics: your vendor now transmits directly to AHCCCS (after certification testing), and you pay for that vendor yourself. Visits captured but never certified into the new pipeline simply don't exist for claims validation.
Any denial cluster that starts around October 2025 should be triaged as a transition problem first: did the vendor complete AHCCCS testing, and is the direct feed live for your provider IDs?
The retiree-state math
Arizona's aging population keeps home-care volume growing faster than the national average, which compounds any percentage-level mismatch into serious dollars. The agencies that treated the EVV 2.0 cutover as a project — vendor certified, feeds tested, visits reconciled — kept their October revenue. The ones that assumed continuity found out the aggregator they were compliant with no longer existed.
